Singles out one especially extreme or striking example among several to comment on it, usually with surprise, criticism, or emphasis. It picks the worst/most notable case and highlights just how far it goes. Often follows a general statement, then zooms in on the standout instance.

Everyone was late, but as for him, he showed up a full hour behind.
に至っては singles out the most extreme offender, 彼, to stress how bad his case was.
成績が下がる生徒が多く、この子に至っては全く勉強しない。
Seiseki ga sagaru seito ga ōku, kono ko ni itatte wa mattaku benkyō shinai.
Many students' grades are dropping, and as for this child, he doesn't study at all.
に至っては highlights the most extreme case among the failing students.
物価が上がり、野菜に至っては去年の倍の値段だ。
Bukka ga agari, yasai ni itatte wa kyonen no bai no nedan da.
Prices have risen, and as for vegetables, they now cost double what they did last year.
に至っては picks out vegetables as the most striking example of the price surge.
• Register: formal/written, often carries criticism or astonishment. • Typically follows a broader statement, then focuses on the standout worst/most extreme case. • The comment after it usually stresses degree (e.g. 全く, 倍, さえ).
The JLPT rarely asks whether you recognise a pattern — it asks which of several similar patterns fits. These are the ones ~に至っては is tested against.
といえば simply introduces a topic by association; に至っては specifically flags the most extreme case, usually negatively.
Plain は just marks a topic for contrast; に至っては adds strong emphasis that this one case is exceptionally extreme.
