Signals contrast or concession — 'although; even though; despite.' Unlike the simultaneous-action ながら, this ながら(も) links two facts that seem contradictory, showing that despite one being true, the other still holds. The も adds emphasis to the surprise of the contrast.

彼は貧しいながらも、幸せに暮らしている。
Kare wa mazushii nagara mo, shiawase ni kurashite iru.
Although he is poor, he lives happily.
い-adjective 貧しい + ながらも contrasts poverty with happiness
残念ながら、今回は参加できません。
Zannen nagara, konkai wa sanka dekimasen.
Regrettably, I can't participate this time.
な-adjective stem 残念 + ながら, a set concessive phrase
悪いと知りながらも、嘘をついてしまった。
Warui to shirinagara mo, uso o tsuite shimatta.
Even though I knew it was wrong, I ended up lying.
知り (stem of 知る) + ながらも contrasts knowing with acting otherwise
• Concessive ながら(も) means 'although,' distinct from the 'while doing' ながら (which needs two actions by one person). • Attaches to verb stems, い-adjectives, な-adjective stems, and nouns. • Register: slightly formal/literary; the も heightens the contrast.
The JLPT rarely asks whether you recognise a pattern — it asks which of several similar patterns fits. These are the ones ~ながら(も) is tested against.
Simultaneous ながら means 'while doing two things at once'; ながらも/concessive means 'although,' expressing contradiction.
のに strongly conveys surprise or complaint; ながらも is more measured and literary, simply noting the contrast.
