一か月に (per month) 一回 (once)
• Pattern: [time period] + に + [number] + 回/度. • Detail: 一日に一回 (once a day), 一週間に二回 (twice a week), 一ヶ月に三回 (three times a month). • Summary: 度 and 回 both mean "times," but 度 is slightly more formal.
The JLPT rarely asks whether you recognise a pattern — it asks which of several similar patterns fits. These are the ones [Period]に[Frequency]回 is tested against.
毎〜 means 'every ~' (毎日 = every day) without specifying how many times; [period]に[N]回 gives the explicit rate/frequency within that period.
